Both factions and nightfall have starter islands and you can do some of the quests but mainly all you want to focus on after doing some early side quests is main story quests (primary quests) only. Other wise, you really should start in factions or nightfall (factions is my favorite). If you want to get everything there is a title for reaching level 20 in pre searing called legendary defender of ascalon and it is a massive grind fest and you should look up guides. If you start in prophecies you can check "pre searing quests" and play around with those to get some skills. Most people never do 90% of them because they don't offer anything most of the time you can't get anywhere else and they take you out of your way and hold you back from all the end game content you could other wise be doing. The amount of side quests in guild wars is actually crazy.
